Membership Discount for Additional Members
Receive an additional 15% off on any household members that sign up through your membership.
Your benefits run for one full year (12 months) from the date this document is signed, must be used in that time period, and are non-transferable to any other party. Unused services expire at the end of the term and cannot be rolled over. You can receive a full refund up to 30 days after purchase if no services or discounts have been used.
Your membership plan payments must be current to receive the services and discounts included in the membership plan. Payment for treatment not included in the membership plan is due at the time of service. It is the responsibility of the member to maximize benefits by scheduling all necessary appointments within the 12-month membership period. In addition, if any household members sign up under our discount program, they will receive 15% off their membership fee. This program can only be used at West Grand Dental in Hot Springs, Arkansas. Discounts under this membership DO NOT apply to any prior treatment.

- 25% off Scaling and Root Planing.
